Integration Care Facilitator

The Integration Care Facilitator plays a vital role in the Program Team, working closely with consumers to provide comprehensive support services. This position requires strong communication and organizational skills, as well as the ability to work effectively in a fast-paced environment.
Key Responsibilities- Conduct outreach and engagement activities to connect with the target population.
- Perform intake interviews and assessments to determine program eligibility.
- Develop and implement individualized service plans (ISPs) to meet consumer needs.
- Collaborate with other providers and entities to ensure seamless service delivery.
- Provide education and support to consumers on various topics, including employment readiness and crisis intervention.
- Assist with housing search and placement, and provide landlord advocacy and tenant stability support.
- Ensure fidelity in the provision of evidence-based practices and services.
- Bachelor's degree in behavioral sciences or a related field.
- Minimum of two years of experience working with homeless, incarcerated, or substance-abuse affected populations.
- Strong working knowledge of computers, electronic health records, and data collection.
- Valid driver's license for business-related travel.
The work environment is fast-paced and dynamic, with a focus on providing high-quality services to consumers. The successful candidate will be able to work effectively in a team environment and adapt to changing priorities and deadlines.
Physical Requirements- Ability to climb three flights of stairs to respond to emergencies.
- Ability to quickly move throughout the facility to respond to emergencies.
- Ability to perform emergency-type actions as needed.
