Primary Responsibilities:
- Providing 245d health care services to clients in their homes or communities.
- Supporting clients with activities of daily living (ADLs) and instrumental activities of daily living (IADLs).
- Collaborating with other healthcare professionals to ensure high-quality care.
- Maintaining accurate records and documentation.
Requirements:
- Associate's degree or higher in a related field or equivalent experience.
- Experience working with people with disabilities and 245d requirements.
- Strong communication and interpersonal skills.
- Ability to pass a background check and obtain necessary certifications.
