Key ResponsibilitiesBehavioral Health Support
  • Conduct brief assessments and screenings to evaluate eligibility and treatment needs.
  • Assist clients with daily living activities and parenting support, fostering positive relationships and routines.
  • Help clients set and achieve personalized goals outlined in their treatment plans.
  • Provide crisis intervention as necessary.
  • Facilitate psychoeducational groups and substance use support sessions.
Case Management Duties
  • Guide clients in accessing essential community resources, including mental health services and vocational training.
  • Document all interactions with clients, families, and community resources.
  • Develop service plans that specify goals and actions to meet client needs, ensuring active participation.
  • Submit progress reports to referring agencies with ongoing recommendations.
  • Perform additional responsibilities as assigned.
