Care Coordinator

Bloom Healthcare is a pioneering and employee-owned primary care practice that delivers innovative medicine to vulnerable patients in the comfort of their own homes.
As a Care Coordinator, you will play a vital role in providing comprehensive primary care to patients with chronic conditions. Your responsibilities will include delivering home visits to 10 patients per day, providing data-driven specialty visits as needed, and supporting short and long-term coverage needs for Bloom NP teammates. You will also engage with population health data to drive superior patient outcomes and commute to assigned assisted living communities to provide care at home.Key Responsibilities:
Deliver comprehensive primary care at home visits to 10 patients per day
Provide data-driven, condition-specific specialty visits to patients as needed
Support short and long-term coverage needs for Bloom NP teammates
Provide on-demand visits to patients with urgent conditions
Demonstrate exceptional communication and relationship-building skills with care team members
Engage with and appropriately utilize population health data to drive superior patient outcomes
Commute to assigned assisted living communities to provide care at home
