Patient Care Coordinator

About the Role:
The Patient Care Coordinator will play a vital role in driving consistency and efficiency in our healthcare operations. This individual will work closely with our clinical staff to identify areas for improvement and implement solutions to enhance patient care and quality outcomes.
Key Responsibilities:
* Motivate providers to engage in quality improvement initiatives to achieve 4-star ratings
* Provide guidance on accurate coding and clinical documentation to ensure high-quality data
* Track and analyze ER/Hospital Admissions and Discharges to inform quality improvement efforts
* Participate in meetings to discuss STAR metrics and provider group performance
* Develop and implement user-friendly initiatives to support practice success
Requirements:
* 1+ years of experience in healthcare and/or customer service
* Strong communication and presentation skills
* Attention to detail, organization skills, and time management skills
* Solid problem-solving skills and ability to interact with medical staff, peers, and internal company staff
* Ability to maintain patient confidentiality and adhere to HIPAA and OSHA requirements
Preferred Qualifications:
* 1+ years of HEDIS experience and/or knowledge
* Experience with medical insurance and/or coding
* Knowledge of clinical standards of care, preventative health, and Star measures
* Knowledge of the Medicare and Medicaid market
* Skills in Microsoft Office, Excel, Outlook, and EMR systems
