Care Coordination Support Role

As a Care Coordination Support Specialist at VNA Health Care, you will play a vital role in ensuring the smooth operation of our Care Coordination team. Your primary responsibility will be to provide administrative support to the Care Coordination staff, enabling them to focus on delivering exceptional patient care.

Key Responsibilities:
  • Respond to customer inquiries and provide timely support to the Care Coordination team.
  • Manage and maintain accurate patient documentation, including electronic medical records and other reporting systems.
  • Coordinate the flow of patient-related documents and correspondence, ensuring timely and efficient processing.
  • Communicate effectively with hospitals, on-call staff, and Care Coordination team members to ensure seamless patient care.
  • Assist in scheduling and organizing departmental meetings, as well as coordinating external patient appointments.
  • Support Care Coordination projects and initiatives as needed.
  • Process office supply requests and serve as a translator for the Care Coordination team.
  • Work collaboratively with the Care Coordination team to meet the needs of patients and families.

Requirements:
  • High school diploma or equivalent, with some college coursework in a business-related field.
  • Knowledge of Microsoft applications, including Word and Excel.
  • Excellent communication and interpersonal skills, with the ability to work effectively with diverse stakeholders.
  • Minimum of 3 years of experience in an office support role, with a background in medical clerical work and medical terminology preferred.
  • Bi-lingual in Spanish/English, with the ability to read, write, and speak fluently.

Essential Functions/Physical Requirements:
  • Ability to communicate effectively verbally and in writing.
  • Ability to drive a car.
  • Manual dexterity sufficient to operate a computer.
  • Ability to sit for prolonged periods of time.
  • Ability to wear a particulate mask.
