You'll work collaboratively with a multidisciplinary team to provide care to patients in their homes, nursing homes, assisted living facilities, and senior housing.
The Senior Community Care program is a key component of our integrated care delivery model. As part of this program, you'll join an esteemed team of professionals dedicated to providing exceptional care to our most vulnerable patients.
Key Responsibilities:
Your primary responsibilities as a Senior Community Care Nurse Practitioner will include:
- Conducting thorough assessments to identify patients' needs and develop personalized care plans.
- Managing patients through acute medical situations or exacerbations of chronic conditions, including ordering tests, conducting diagnostics, and implementing interventions.
- Providing care during short-term sub-acute rehabilitation episodes, focusing on restoring patients' functional abilities.
- Maintaining accurate and complete documentation, adhering to ICD 10 coding standards.
- Collaborating with the interdisciplinary care team to ensure seamless care coordination, minimizing fragmentation and optimizing patient outcomes.
- Counseling patients and their families on advanced illness and end-of-life care options, empowering them to make informed decisions about their care.
Requirements:
- Hold a graduate degree from an accredited Master of Science Nursing or Doctor of Nursing Practice program.
- Be licensed as an APC in New Jersey or have the ability to obtain licensure by start date.
- Possess active Nurse Practitioner certification through a national board, such as the American Academy of Nurse Practitioners or the American Nurses Credentialing Center, with certification in one of the following: Family Nurse Practitioner, Adult Nurse Practitioner, Gerontology Nurse Practitioner, or Adult-Gerontology Acute Care Nurse Practitioner.
- Hold current, active DEA licensure/prescriptive authority or be able to obtain post-hire, per state regulations (unless prohibited in your state).
- Be able to secure a collaborative practice agreement, if applicable in your state.
- Meet the physical requirements of the job, including lifting a 30-pound bag in and out of the car, navigating stairs, and performing physical assessments.
- Have reliable transportation.
Preferred Qualifications:
- 1+ years of clinical experience, preferably in long-term care settings.
- Experience in meeting the medical needs of patients with complex behavioral, social, and functional needs.
- Understanding of Geriatrics and Chronic Illness.
- Knowledge of Advanced Illness and end-of-life discussions.
- Proficient computer skills, including documenting medical information in electronic medical records.
New Jersey Residents Only: $88,000 - $173,200 annually, Pay is based on factors such as local labor markets, education, work experience, certifications, and more. UnitedHealth Group complies with all minimum wage laws as applicable.
