Responsibilities As a Case Manager Coordinator, you will be responsible for:

  • Serving as a system navigator and point of contact for shared CCBHC/CCDOT clients and families;
  • Assisting new clients with completing paperwork and releases of information needed;
  • Negotiating on behalf of clients with treatment teams and patients themselves to create reasonable goals and clarify issues;
  • Maintaining accurate records of caseload and completing notes for all care coordination services;
  • Providing weekly data to managers and following up with programs on no-shows, client engagement issues, and increased use of or need for services.
