Overview of the Role

This position involves providing clinical care and assessments to patients in their homes, as part of Optum's HouseCalls product. The role requires a Nurse Practitioner with strong clinical skills and the ability to work independently.

About the Job

The successful candidate will be responsible for conducting comprehensive assessments of patients, including evaluations of past medical history, review of systems, medication reconciliation, vital signs, and physical exams. They will also perform evidence-based practice screenings, identify diagnoses, and formulate lists of current and past medical conditions.

In addition, the candidate will communicate findings to inform members' primary care physicians of potential gaps in care and identify urgent and emergent situations, intervening appropriately. They will also educate patients on disease processes, medications, and compliance.

Key Responsibilities
  • Conduct in-home assessments, including comprehensive physical exams and evidence-based practice screenings
  • Identify diagnoses and formulate lists of current and past medical conditions
  • Communicate findings to inform members' primary care physicians of potential gaps in care
  • Educate patients on disease processes, medications, and compliance
Requirements

To be successful in this role, candidates must have an active NP license or be able to obtain one by start date, with licensure free of any open/unresolved disciplinary actions. They should also have obtained approval to practice autonomously or without formal supervision from their licensing board, if applicable.

Candidates must also have active ANCC or AANP national certification in Family, Geriatrics, Adult, Adult-Gerontology Primary Care, or Adult-Gerontology Acute Care certification, or be able to obtain it by start date. Additionally, they must be able to complete physical requirements of the job, including lifting a 30-pound bag, navigating stairs, and sitting, standing, and kneeling to perform physical assessments.

Preferred Qualifications
  • 1+ years of clinical experience in their highest level of education, clinical setting
  • Experience in Gerontology, Cardiology, Internal medicine, or Endocrinology
  • Home Health care or home visit experience
Compensation and Benefits

The salary range for this role is $88,000 to $173,200 annually, based on several factors including local labor markets, education, work experience, certifications, etc. UnitedHealth Group complies with all minimum wage laws as applicable. In addition to salary, employees receive a comprehensive benefits package, incentive and recognition programs, equity stock purchase, and 401(k) contribution (all benefits are subject to eligibility requirements).
