About the Role

We are seeking an experienced Nurse Practitioner to join our team as a Geriatric Care Specialist, providing high-quality care to seniors in long-term care settings across New Jersey. This role requires travel throughout the state, working collaboratively with healthcare providers to deliver exceptional patient care.

Key Responsibilities

  • Conduct comprehensive assessments and develop individualized care plans for patients
  • Provide medical history reviews, diagnose conditions, and develop treatment plans
  • Collaborate with healthcare teams to ensure seamless care coordination
  • Identify risk factors and develop interventions to mitigate barriers to accessing care
  • Communicate effectively with patients, families, and healthcare providers to promote informed decision-making

Requirements

  • Active APC license in New Jersey or ability to obtain
  • Board certification through AANP or ANCC (Family, Adult, or Gerontology NP)
  • Current DEA licensure/prescriptive authority or ability to obtain post-hire
  • Ability to travel 75% - 100% time, including overnight trips
