**Job Summary**
InGenesis is seeking a highly skilled Customer Service Liaison to join our team. As a key member of our client's support team, you will play a critical role in ensuring the accuracy of information collected and communicating coordination of care information with team members, patients/caregivers, referral sources, and home health agencies.
**Key Responsibilities:**
- Supports several local intake teams on new patient referrals, ensuring the accuracy of information collected.
- Communicates coordination of care information with team members, patients/caregivers, referral sources, and home health agencies.
- Maintains organized file systems for document tracking and patient management/follow-up.
- Ensures compliance with applicable State and/or Federal laws, accrediting requirements, and company policies.
- Maintains confidentiality of patient information.
- Works independently on other projects as assigned.
- Completes company-mandated and position-specific training assignments timely and according to policy.
- Performs other duties as assigned.
**Requirements:**
- High school graduate or GED required.
- 0 to 1 year of related experience.
- Previous call center experience preferred.
- Proficiency with Microsoft Office Suite and keyboarding skills.
- Knowledge of computerized information systems.
- Ability to define problems, collect data, establish facts, and draw valid conclusions.
