Job Title: Housing Liaison

At Prestige Healthcare Resources, Inc., we are seeking a highly motivated and compassionate Housing Liaison to join our team. This is an excellent opportunity to make a meaningful difference in the lives of our clients and contribute to the growth and success of our organization.

Job Summary:

The Housing Liaison will provide comprehensive support services to clients residing in our supportive housing program. This includes outreach, case management, counseling, advocacy, and other needed services to ensure the well-being and stability of our clients.

Key Responsibilities:
  1. Provide individualized support to clients, including developing and implementing case management plans that address their unique needs and goals.
  2. Collaborate with internal and external stakeholders, including building managers, clinical staff, and leadership, to ensure seamless communication and coordination of services.
  3. Document client progress, behavior, incidents, and discussions with the Director of Clinical Services and House Manager Supervisor.
  4. Work with clients to develop and maintain stable housing, address functional impairments, and engage in ongoing treatment and support services.
  5. Educate participants on good housekeeping and home-safety practices, budgeting, and landlord/tenant relations as necessary.
Requirements:
  1. At least 1-3 years of work experience in social services, supported housing, counseling, or peer support, including crisis management, case management, and rehabilitation supports.
  2. Strong communication and interpersonal skills, with the ability to work effectively with diverse populations and stakeholders.
  3. Ability to maintain confidentiality and handle sensitive information with discretion.
  4. Flexibility and adaptability in a fast-paced environment with changing priorities and deadlines.

Work Environment:

This role will work in a professional office environment and in the field/community, with routine use of standard office equipment.

Physical Demands:

This position does not involve local travel on a consistent basis.

Position Type and Expected Hours of Work:

This is a part-time position, working Friday, Saturday, and Sunday from 7 pm to 7 am.
