Health First is seeking a skilled Nurse Specialist to join our team.

POSITION SUMMARY:

To provide high-quality patient care and improve outcomes, the successful candidate will utilize the nursing process to address complex wound care and continence issues, including altered skin integrity, diversion of the urinary and/or gastrointestinal tract, and incontinence of the bowel and/or bladder.

  • Utilize case-finding strategies to identify at-risk patients and provide individualized care plans.
  • Develop and implement educational programs for patients, families, and healthcare staff to promote wound care and continence best practices.

PRIMARY ACCOUNTABILITIES:

Quality/No Harm:

  • Demonstrate advanced nursing knowledge and practice in wound care and continence management, including conservative sharp debridement and ostomy care.
  • Participate in the development and implementation of standards of care, policies, and clinical pathways related to wound care and continence.

Stewardship:

  • Contribute to research initiatives to improve wound care and continence outcomes and participate in quality improvement projects.
  • Collaborate with interdisciplinary teams to develop and implement evidence-based practices.

Customer Experience:

  • Provide clinical consultations and education to healthcare staff and patients to promote wound care and continence best practices.
  • Serve as a resource for patients and families to promote continuity of care and support after discharge.
