Cape Abilities seeks a dedicated Community Health Worker to join our team.

About the Role

As a Community Health Worker, you will provide administrative support to program case management and work closely with Nurses and Case Managers to ensure efficient delivery of services. Your key responsibilities will include:

  • Providing home visit relief when needed;
  • Holding a small case load and performing Case Manager duties for that caseload;

Additionally, you will be responsible for chart audits, completing identification sheets and photos, tracking six-month summaries and physician orders, and maintaining updated records.

Requirements

  • High school diploma;
  • Valid driver's license in Massachusetts;
  • Proficiency in PC applications;
  • Ability to work under pressure and multitask;

Working Conditions/Physical Demands

  • Lifting up to 20 pounds for office supplies.

Salary Range: $35,000 - $45,000 per year
