Job Summary

We are seeking a dedicated and detail-oriented Coordinating Care for Transplant Patients to join our team at Chi. This is a full-time position that offers a competitive salary of $95,000 - $125,000 per year, depending on experience.

About the Role:

  • Coordinate the referral and evaluation process for potential transplant candidates.
  • Review medical information, schedule testing, and collect data for presentation to the Medical Review Board.
  • Maintain effective communication with referring physicians and third-party payers.
  • Initiate and oversee pre-op and post-op education for patients.
  • Screen potential donor calls and coordinate donor/recipient status at the time of transplant.

Requirements:

  • Bachelor's Degree in Nursing.
  • Registered Nurse (RN) license.
  • Basic Life Support (BLS) certification through the American Heart Association.
  • Three years' experience in acute care/ICU and/or transplant experience.
