Job Title: Nursing Transplant Team Lead

Baylor St. Luke's Medical Center collaborates with Texas Heart Institute and Baylor College of Medicine to drive innovation in cardiovascular care, neuroscience, oncology, and transplantation.

The McNair Campus prioritizes patient safety, staff safety, and physician safety through evidence-based practices.

We are seeking an experienced Nursing Transplant Team Lead to oversee transplant services and ensure high-quality patient care.

Responsibilities:

  • Lead transplant services to guarantee exceptional patient care.
  • Facilitate referrals and evaluations for potential transplant candidates.
  • Review medical information, schedule testing, collect data, and present patient cases at Medical Review Board meetings.
  • Develop and implement daily care plans for in-hospital patients, ensuring effective communication with patients, families, and healthcare teams.
  • Maintain open communication with referring physicians and third-party payers.
  • Collaborate with nursing staff to create comprehensive teaching plans for pre-op and post-op education.
  • Evaluate learning needs and provide personalized instruction.
  • Manage potential donor calls, coordinate donor/recipient status, and maintain accurate statistics.

Requirements:

  • Bachelor's Degree - Nursing
  • Registered Nurse (RN)
  • Basic Life Support (BLS) certification through the American Heart Association (AHA)
  • Minimum three years' experience in acute care/ICU and/or transplant setting
  • Exceptional written and verbal communication skills
  • Proven problem-solving abilities

Salary:

This role offers a competitive salary range of $120,000 - $150,000 per year and is based in Houston, TX.
