Nurse Case Manager

Capital Hospice dba Capital Caring is seeking a highly skilled Registered Nurse Case Manager to join our team. As a key member of our interdisciplinary team, you will provide comprehensive nursing care to our patients and their families, while also coordinating and managing the nursing component of our hospice caseload.
Responsibilities- Conduct initial and ongoing nursing assessments, judgments, and treatment decisions based on patient and family needs and wishes
- Develop and implement individualized care plans in collaboration with the interdisciplinary team
- Supervise and mentor LPNs, CNAs, and Patient Care Volunteers
- Collaborate with physicians, payers, and other healthcare professionals to ensure seamless care coordination
- Provide education and support to patients and families on end-of-life care and related topics
- Current licensure as a Registered Nurse in the District of Columbia, State of Maryland, and/or Commonwealth of Virginia
- At least one year of staff nurse experience in a comparable setting/unit
- Hospice and/or palliative care experience preferred
- BSN degree preferred
- Healthcare Provider BLS (CPR) certification required
- Excellent communication and interpersonal skills
- Ability to work independently and as part of a team
- Strong organizational and time management skills
- Ability to manage multiple tasks and priorities in a fast-paced environment
- Proficiency in Microsoft Office and patient health information systems
