Job Summary:
The Registered Nurse Hospice Case Manager provides comprehensive nursing care to patients and families in a hospice setting. This role requires collaboration with the interdisciplinary team to develop and implement individualized care plans that meet the unique needs of each patient.
Responsibilities:
- Conduct initial and ongoing assessments of patients and families to identify their needs and develop care plans that address those needs.
- Collaborate with the interdisciplinary team to develop and implement care plans that meet the unique needs of each patient.
- Provide education and support to patients and families on disease management, symptom control, and end-of-life care.
- Supervise and coordinate the care of hospice patients and families, including the management of medications, treatments, and other interventions.
- Document patient care and progress in accordance with regulatory requirements and company policies.
Qualifications:
- Must have a degree in Nursing from an accredited nursing program.
- Must have licensure from the governing body for practice for the District of Columbia, State of Maryland, and/or the Commonwealth of Virginia, based on the state/jurisdiction of practice.
- Must have one year of staff nurse experience in a comparable setting/unit.
- Hospice and/or palliative care experience is strongly preferred.
Work Environment:
This role requires working in a hospice setting, providing care to patients and families in their homes or in a facility. The Registered Nurse Hospice Case Manager must be able to work independently and as part of a team to provide comprehensive care to patients and families.
