Job Summary
We are seeking a highly skilled Registered Nurse Care Manager to join our team. As a RN Care Manager, you will work closely with complex renal patients in their homes, dialysis centers, and virtually to improve patient outcomes. Your primary focus will be to help patients achieve permanent access, promote home dialysis modalities, and educate patients on self-management.
Responsibilities
- Conduct comprehensive assessments of patients' medical, behavioral, pharmaceutical, and social needs to identify gaps in care and barriers to good health.
- Create and implement care plans that address identified needs and improve patient health.
- Coordinate care by serving as a patient advocate and resource for patients, families, and healthcare providers.
- Facilitate care across the continuum of care, including home, hospital, skilled nursing facility, and acute care settings.
- Manage patients during transitions of care to minimize avoidable readmissions.
- Provide ongoing reassessment and follow-up to improve patient outcomes.
Requirements
- 1+ years of nursing experience in case management or care management, preferably coordinating care across multiple settings.
- 2+ years of healthcare-related experience.
- Current, unrestricted compact Registered Nurse license.
- Requires all teammates to maintain current, valid BLS certification.
- Core values consistent with a patient-centered approach to care.
Preferred Qualifications
- Bachelor's degree in nursing.
- Demonstrates empathy, enthusiasm, a great sense of humor, and a strong work ethic.
- Experience working with vulnerable patient populations.
- Ability to establish rapport with patients and families.
