Job Summary

We are seeking a skilled and compassionate Healthcare Referral Specialist to join our team at Los Angeles Christian Health Centers. As a key member of our healthcare team, you will play a vital role in helping clients access necessary community resources and services.

Key Responsibilities
  • Process referrals for subspecialty, diagnostic, and mammogram services, communicating with outside agencies and healthcare providers to ensure seamless coordination.
  • Notify patients of appointment times, educate them on the significance of their appointments, and provide reminder calls to ensure they follow through with their scheduled appointments.
  • Track the status of referrals and maintain accurate records, including referral logs and client encounter documentation.
  • Prepare and submit monthly reports of activities as requested.
  • Research and update outside referral sources on an ongoing basis.
  • Communicate and share information with Patient Navigators from other LACHC service sites.
  • Serve as a liaison between patients and medical staff for both internal and external services.
  • Educate clients on available benefits and refer them to Eligibility Specialists to assist with obtaining such benefits.
  • Assist with preparation of forms and implementation of outreach strategies for various screening events.
  • Provide patient education on breast health awareness and review patient charts prior to releasing to requesting providers or departments.
Requirements
  • High School Diploma or GED required.
  • Bachelor's degree in psychology, social work, or a related field from a four-year college or university, or 2 years of work experience, or an equivalent combination of education and experience required.
  • Prior case management experience preferred.
  • Prior experience working with the homeless, substance using, or chronically mentally ill individuals preferred.
Preferred Qualifications
  • Experience with Microsoft Windows, Microsoft Access, and Microsoft Excel preferred.
  • Familiarity with medical terms and operations of clinics useful.
  • Proficiency in English required, Bi-lingual/Bi-literal Spanish highly preferred.
