About the Role

We are seeking a skilled RN Care Coordinator to join our team at Chapters Health System. As a key member of our Interdisciplinary Group, you will play a vital role in providing high-quality patient care and support to individuals and their families.

Key Responsibilities
  • Assess and identify patient/family needs, utilizing the nursing process to develop and implement individualized plans of care.
  • Coordinate with the Interdisciplinary Group to ensure seamless care delivery and effective communication.
  • Provide palliative and supportive care to patients and their families, addressing physical, emotional, and spiritual needs.
  • Collaborate with healthcare professionals to ensure timely and efficient care transitions.
  • Document patient information and communicate effectively with the care team.
Requirements
  • Current RN license in the state where the employee will be working.
  • Minimum one year of in-patient nursing or hospice experience.
  • Previous experience working with an EMR/EHR system.
  • Valid driver's license and automobile insurance.
  • Ability to use equipment with visual and auditory mechanisms.
  • Effective communication skills in English (verbal and written).
  • Flexible schedule to meet the needs of Hospice House patients.
