As a RN, Care Coordinator with Chapters Health System, you will play a vital role in providing exceptional patient care and support to individuals and their families. Key Responsibilities- Patient Assessment and Care Planning: Conduct thorough assessments of patients' needs, develop and implement individualized care plans, and coordinate with the Interdisciplinary Group (IDG) to ensure comprehensive care.
- Communication and Education: Educate patients, families, caregivers, and healthcare professionals about disease processes, palliative interventions, care giving, and dying processes.
- Collaboration and Teamwork: Work closely with the IDG to evaluate hospice referrals and admissions, attend patient deaths, and prepare bodies for transport.
- Bereavement Support: Provide bereavement interventions to families and caregivers in conjunction with psychosocial staff.
- Documentation and Reporting: Maintain accurate and timely documentation, present concise reports to the IDG, and communicate effectively with physicians, staff members, patients, families, and supervisors.
- Leadership and Supervision: Supervise patient care given by other nursing team members and perform other duties as assigned.
- Education and Licensure: Current license as RN in the state where the employee will be working.
- Experience: Minimum one (1) year of in-patient nursing or hospice experience for external candidates; for internal applicants, minimum of 6-months of LPN experience with hospice/home health within an affiliate.
- Skills and Abilities: Ability to use equipment with visual and auditory mechanisms, ability to effectively communicate in English (verbal and written), and ability to manage the emotional stress of working with and caring for terminally ill patients and their families.
- Physical Demands: Ability to perform the essential functions and physical requirements of the job with or without reasonable accommodation, including standing, sitting, reaching, lifting, bending, and climbing stairs.
