Job Summary

The Care Coordinator RN Sr is a key member of the healthcare team, responsible for ensuring seamless transitions of care for patients. This role requires strong clinical expertise, effective communication skills, and a deep understanding of the healthcare system.

Key Responsibilities
  • Assess transitional care needs and coordinate care across the continuum
  • Partner with physicians and care teams to drive optimal and efficient treatment plans

  • Mentor new hires in clinical progression and case reviews
Requirements
  • Bachelor's degree in Nursing
  • ACM or CCM certification
  • Minimum 3 years of experience as a staff nurse in an acute care hospital setting
  • Minimum 2 years of experience as a case manager in a hospital setting or payer-based model
Preferred Qualifications
  • Knowledge of local, state, and federal regulations
  • Knowledge of community and state-wide resources and programs
