Job Summary

We are seeking a highly motivated and detail-oriented Case Management Assistant to join our team at Salem Hospital, Inc. This role will provide essential support to our Case Manager in coordinating, negotiating, and managing resources to ensure efficient and effective patient care.

Key Responsibilities
  • Assist the Case Manager in coordinating resource utilization within the continuum of care
  • Work closely with members of the care team to ensure seamless communication and collaboration
  • Support the Case Manager in facilitating achievement of quality care and patient cost outcomes
  • Participate in quality improvement and evaluation processes related to patient care
  • Provide essential support to the Mission of Salem Hospital and goals for high-quality care
Requirements
  • Mature professional with high level of personal energy and professional motivation
  • Knowledge of payer requirements, vendor services, and systems and processes related to obtaining those services
  • Commitment to cost-effective patient care and patient education
  • Excellent interpersonal and communication skills
  • Computer literate with ability to quickly gain competency in various hospital software applications
Qualifications
  • High school diploma or equivalent required
  • One or more years of experience in a service/healthcare industry environment
