Job Summary

Mass General Brigham is seeking a highly skilled Registered Nurse to lead our Case Management Program at North Shore Medical Center.

The ideal candidate will have a strong background in nursing and case management, with experience in planning, directing, and managing programs. They will be responsible for developing and supervising staff, evaluating and maintaining case management skills and outcomes, and ensuring adherence to key model components.

The successful candidate will work collaboratively with the multidisciplinary team to provide quality, timely, and cost-efficient care to patients and their families. They will also be responsible for communicating complex information in a professional and courteous manner, and for demonstrating strong analytical and problem-solving skills.

Responsibilities
  • Develop and implement case management programs and services
  • Supervise and develop staff, including RNs, SWs, and non-clinical support
  • Evaluate and maintain case management skills and outcomes
  • Manage all policies and procedures related to case management
  • Ensure adherence to key model components
  • Collaborate with the multidisciplinary team to provide quality, timely, and cost-efficient care
  • Communicate complex information in a professional and courteous manner
  • Demonstrate strong analytical and problem-solving skills
Requirements
  • Minimum 7 years acute clinical experience
  • RN with Master's Degree
  • Current License as Registered Nurse in the Commonwealth of Massachusetts
  • Minimum 5+ years of progressive management experience with demonstrated goal-oriented outcomes
  • Demonstrated leadership and change management experience
  • Case Management experience preferred
