Case Manager RN Job Description

Mass General Brigham is seeking a skilled RN Case Manager to join our experienced Case Management team. This role focuses on transition planning and Utilization Review, with a goal of assessment and development of initial transition plans.

Key Responsibilities:

  • Initial assessment and development of transition plans
  • Participation in multidisciplinary rounds
  • Utilization review
  • Patient advocacy
  • Ability to delegate tasks to Case Management assistants

Requirements:

  • High degree of skill in written/oral communications and personal interactions
  • Three or more years of clinical experience in a health care delivery system
  • Completion of an accredited Nursing program
  • Current Nursing License in the State of MA is required

Preferred:

  • Case Management, Home Care Discharge Planning experience
