Hospice RN Case Manager Job Summary

Interim HealthCare Staffing is seeking a compassionate and skilled RN Case Manager to join our team. As a Hospice RN Case Manager, you will provide exceptional care to patients and families, upholding our high standards of nursing practice.

Key Responsibilities:
  • Maintain a specified case management load.
  • Conduct initial and ongoing assessments of patients' physical and psychosocial status.
  • Develop and implement individualized plans of care in collaboration with the Interdisciplinary Group (IDG).
  • Coordinate care among IDG members and other healthcare providers to ensure continuous assessment and implementation of the IDG plan of care.
  • Direct and supervise services provided by LPN/LVN, Hospice Aide, and Homemaker.
  • Evaluate and coordinate the need for services provided to patients and families.
Requirements:
  • Graduate of an accredited school of nursing.
  • Licensed to practice as a RN in the state of South Carolina.
  • Twelve (12) months of accumulated experience as a registered nurse within the last five (5) years or, if no experience within the last five (5) years, evidence of current clinical knowledge.
  • One (1) year of hospice care experience.
  • One (1) year of precepting or training experience preferred.
  • CPR certification.
