Job Summary

We are seeking a skilled and compassionate Registered Nurse to join our team at WVUHS Home Care, LLC. As a Home Health Nurse, you will provide high-quality patient care in the comfort of their own homes, working collaboratively with our interdisciplinary team to develop and implement personalized care plans.

Key Responsibilities
  • Provide direct and indirect nursing care to patients in their homes, including assessments, treatments, and education.
  • Collaborate with patients, families, and healthcare teams to develop and implement care plans that meet individual needs and goals.
  • Conduct thorough assessments of patients' physical and psychosocial needs, identifying areas for improvement and developing strategies for care.
  • Administer medications and treatments as ordered by physicians, ensuring accurate and timely delivery.
  • Document patient progress and care provided, maintaining accurate and detailed records.
  • Communicate effectively with patients, families, and healthcare teams, ensuring seamless care coordination and transitions.
  • Participate in ongoing education and training to stay current with best practices and industry developments.
Requirements
  • Current licensure as a Registered Nurse in the state of West Virginia or ability to obtain licensure.
  • Valid driver's license and reliable transportation.
  • One year of experience in home health nursing or a related field.
  • Strong communication and interpersonal skills, with ability to work effectively with diverse populations.
  • Ability to lift, move, and position patients of all weights, with varying amounts of assistance.
  • Physical stamina to perform job duties, including frequent walking, standing, and bending.
