Job Summary

We are seeking a highly skilled and compassionate Registered Nurse to join our Home Care team at WVU Medicine. As a Home Health Nurse, you will provide high-quality patient care in the comfort of their own homes, working collaboratively with patients, families, and healthcare teams to develop and implement individualized care plans.

Responsibilities
  • Provide direct and indirect nursing care to patients in their homes, including assessment, planning, implementation, and evaluation of care.
  • Collaborate with patients, families, and healthcare teams to develop and implement individualized care plans that meet the unique needs of each patient.
  • Conduct thorough assessments of patients' physical, emotional, and social needs, and develop plans to address these needs.
  • Implement and evaluate the effectiveness of care plans, making adjustments as needed to ensure optimal patient outcomes.
  • Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination and transitions.
  • Document patient care and progress in a timely and accurate manner, using electronic medical records and other tools as needed.
  • Participate in ongoing education and training to stay current with best practices and evidence-based care.
  • Work collaboratively with other healthcare professionals to ensure comprehensive and coordinated care.
Requirements
  • Current licensure as a Registered Nurse in the state of West Virginia or other states where services will be provided.
  • Valid driver's license and reliable transportation.
  • CPR certification within 30 days of hire date.
  • One year of experience in home care nursing or a related field.
  • Ability to work independently and as part of a team, with strong communication and interpersonal skills.
  • Ability to lift, move, and position patients of all weights, with varying amounts of assistance, as needed.
  • Ability to work in a fast-paced environment, with frequent interruptions and changing priorities.
