As a Clinical Care Coordinator at Catholic Health System, you will be responsible for coordinating nursing care for patients, assessing their needs, and implementing effective care plans. You will work closely with a multidisciplinary team to ensure seamless patient care.

Responsibilities
  • Education and Training
    • You will gain experience under the supervision of a Registered Professional Nurse and maintain professional knowledge and skills through educational conferences and training programs.
    • You must be licensed in New York State and graduate from an accredited school of nursing.
    • You will pass the New York state licensing boards within four months of hire.
    • You must also have BLS and ACLS validation in accordance with AHA standards.
  • Leadership and Teamwork
    • You will assist in directing employees, instruct, assign, and evaluate performance.
    • You will participate in educational conferences, maintain professional knowledge and skills, and adhere to hospital policies.
    • You will conserve supplies, equipment, and time in a cost-effective environment.
  • Working Conditions
    • You will be exposed to communicable diseases and must cope with stress and handle multiple priorities.
