About the Role

As a member of our interdisciplinary team, you will work closely with patients, caregivers, and healthcare professionals to deliver high-quality, patient-centered care in the comfort of their own homes.

  • Conduct comprehensive assessments and develop personalized care plans to meet the unique needs of each patient.
  • Collaborate with primary care physicians, specialists, and other healthcare professionals to ensure seamless care coordination.
  • Provide education and support to patients and their families on disease management, self-care, and wellness strategies.
  • Stay up-to-date with the latest medical research, technologies, and best practices to deliver evidence-based care.
Responsibilities

Perform a range of duties, including:

  • Conducting initial and ongoing assessments of patients' physical and emotional needs.
  • Developing and implementing individualized care plans to address patients' goals and priorities.
  • Providing direct patient care, including administering medications, performing procedures, and managing chronic conditions.
  • Collaborating with healthcare teams to ensure effective care coordination and communication.
Requirements

We are seeking a skilled and compassionate medical professional with:

  • Current state RN license and NP or PA certification.
  • 2+ years of clinical experience in primary care, home-based medical care, geriatrics, or palliative care.
  • Strong knowledge of long-term care, community resources, and cost-effective alternatives.
  • Excellent communication and interpersonal skills.
