We have an exciting opportunity for a Patient Care Coordinator to join our team at Christiana Care As a Patient Care Coordinator, you will be responsible for managing patient care, driving patient progression, and establishing discharge plans. This role involves collaborating with an interdisciplinary team, creating and implementing treatment plans, and ensuring safe, timely, and effective transitions throughout the care continuum.

About the Role

This is a full-time position working three out of four weekends per month on Saturdays and Sundays from 8 a.m. to 8:30 p.m., plus one rotating Friday shift (4 p.m. to 8 p.m.). As part of our Weekend Incentive Program, you will receive a special rate of pay and full benefits, except for Paid Time Off (PTO).

In this role, you will work closely with our Case Management Triad Team Model, which consists of a RN Care Manager, Social Worker, and Utilization Management. Together, we strive to provide exceptional patient care and support discharge needs.

Key Responsibilities
  • Identify patients who require post-acute care, placement, and complex discharge planning based on comprehensive assessments.
  • Develop and implement discharge plans for patients with post-acute care needs, collaborating with physicians, nurses, and other healthcare providers.
  • Monitor patient progress and adjust treatment plans as needed to ensure safe, timely, and effective transitions throughout the care continuum.

Christiana Care offers a competitive salary range of $75,000 - $90,000 annually, depending on experience. Requirements
  • An active RN license in DE or Compact State is required.
  • BSN preferred or commitment to obtaining BSN within three years of hire date.
  • RN with experience in Case Management, Transitional Care, or Discharge Planning in an inpatient hospital are preferred.
  • BLS preferred.
  • Case Management Certification (CCM) is required within 18 months of eligibility.
  • Complete 8 continuing education credits (CEU) per year in Utilization and/or Case Management.
