Role Summary:

The After-Hours Registered Nurse (RN) plays a vital role in delivering compassionate nursing care to patients facing terminal illnesses during evening, overnight, and weekend shifts. This position involves providing support to patients and their families in home environments, offering empathy and guidance during challenging times, and ensuring high-quality care is delivered in the comfort of their homes.

Qualifications for After-Hours Registered Nurse:

  • Graduation from an accredited nursing program
  • Current RN licensure in the state
  • Preferred: Minimum of 2 years nursing experience
  • Preferred: Recent patient interaction within the last three years
  • Ability to work autonomously and make timely, accurate decisions
  • Capability to supervise and guide others effectively
  • Skill in responding to crises outside of hospital settings
  • Acceptance and adaptability to diverse social, cultural, and religious backgrounds

Responsibilities:

  • Conduct regular assessments of patients' nursing needs in their homes
  • Provide nutritional guidance
  • Administer nursing services, treatments, and preventive care
  • Initiate nursing procedures that ensure patient safety and comfort
  • Monitor and report any significant changes in patients' conditions to the medical team
  • Educate and support patients and their families in care provision
  • Oversee and mentor other nursing staff
  • Collaborate with the interdisciplinary team to develop and update care plans
  • Perform specific nursing tasks as per physician's orders
  • Stay informed on organizational updates through active participation in meetings
  • Maintain accurate patient records as required
  • Adhere to confidentiality and HIPAA regulations
  • Communicate with the On-Call Clinical Manager regarding patient needs
  • Demonstrate flexibility in providing after-hours coverage as necessary
  • Work cohesively with the After-Hours Team to meet patient needs
  • Engage in the interdisciplinary approach to patient care
  • Coordinate care for patients in various healthcare settings
  • Manage time effectively to attend required meetings
  • Follow infection control policies and protocols
  • Assist in training and orientation as needed
  • Perform additional duties as assigned by the Director of Clinical Services or Clinical Manager
