The Intake team collaborates with hospitals, healthcare facilities, and community physicians to manage referrals for patients entering Hospice care. Candidates who possess outstanding communication abilities and can effectively handle multiple tasks will excel in this position.
Key Responsibilities of the Intake Nurse
- Serve as a crucial link between referral sources, external organizations, healthcare professionals, and field staff.
- Manage and coordinate hospice admissions, including screening calls and referrals to ensure access to suitable services.
- Engage with patients and their families to discuss the arrangements for the initiation of services.
Compensation
- LPN: starting salary of $50,000
- RN: starting salary of $60,000

Essential Qualifications
- Active, unrestricted license as a Registered Nurse or Licensed Practical Nurse in the State of Michigan – BSN is not mandatory for RN candidates.
- A minimum of one (1) year of clinical experience as a Registered Nurse or LPN.
- Exceptional written and verbal communication skills.
- Ability to manage multiple tasks throughout the day.
Preferred Experience
- At least one (1) year of experience in Home Health or Hospice settings.
