Role Summary:
The After Hours Registered Nurse (RN) is responsible for delivering critical nursing care to patients with terminal illnesses during evening, overnight, and weekend shifts. This role involves providing support to patients and their families in home care environments, offering compassion and guidance during challenging times, and ensuring high-quality care is delivered in the comfort of the patient's home.
Qualifications for After Hours Registered Nurse:
- Graduation from an accredited nursing program.
- Current RN licensure in the state.
- Preferred: Minimum of 2 years of nursing experience.
- Preferred: Active patient interaction within the last three years.
- Ability to work autonomously and make timely, accurate decisions.
- Capability to supervise others effectively.
- Proficiency in responding to crises outside of a hospital environment.
- Acceptance and adaptability to diverse social, cultural, and religious backgrounds.
Responsibilities:
- Conduct regular assessments of patients' nursing needs in their homes.
- Provide dietary guidance.
- Administer hospice nursing services, treatments, and preventive care.
- Initiate nursing procedures tailored to the patient's hospice care and safety.
- Monitor and report any significant changes in patients' physical or emotional conditions to the physician and interdisciplinary team.
- Educate and support patients and their families in managing care.
- Oversee and mentor other nursing staff.
- Collaborate with the interdisciplinary group to develop and revise care plans.
- Perform specific nursing tasks as directed by physician orders.
- Stay informed about organizational updates through active participation in meetings.
- Maintain accurate records as required by hospice regulations.
- Adhere to hospice policies and procedures, ensuring confidentiality and compliance with HIPAA regulations.
- Communicate with the On-Call Clinical Manager regarding any unmet patient needs.
- Demonstrate flexibility in providing after-hours coverage as necessary.
- Work collaboratively with the After Hours Team to address patient needs.
- Engage in a team-oriented approach to patient care.
- Coordinate care plans for patients in various care facilities.
- Manage time effectively to attend all required meetings.
- Follow infection control protocols as per agency guidelines.
- Assist with orientation and training as needed.
- Perform additional duties as assigned by the Director of Clinical Services or Clinical Manager.
