Job Summary

We are seeking a skilled and compassionate Hospice Nurse to join our team in Fort Wayne, Indiana. As a Registered Nurse in this role, you will provide high-quality patient care services to individuals with terminal illnesses, working closely with an interdisciplinary team to ensure seamless care delivery.

Key Responsibilities:

  • Provide direct patient care services, including assessing patient needs and developing individualized care plans.
  • Manage an interdisciplinary team of healthcare professionals, ensuring effective communication and collaboration to achieve optimal patient outcomes.
  • Conduct regular patient assessments, monitoring changes in condition and adjusting care plans accordingly.
  • Promote company core values, participating in staff meetings and adherence to quality standards.

Requirements:

  • Current license to practice nursing in Indiana
  • BLS certification (American Heart Association)
  • Valid driver's license and automobile insurance
  • Minimum one year experience as an RN in acute care, oncology, long-term care, hospice, or home health
