Patient Care Coordinator

As a Patient Care Coordinator - Hospice Setting, you will be responsible for providing high-quality care to patients in their homes or in our hospice facilities. Your duties will include:
- Assisting patients with daily living activities, such as bathing, dressing, and grooming
- Providing emotional support and companionship to patients and their families
- Monitoring patients' conditions and reporting any changes to the healthcare team
- Collaborating with other healthcare professionals to develop and implement comprehensive care plans
- Documenting patient information and maintaining accurate records
Requirements: To be successful in this role, you must have a current CNA certification and at least six months of experience working in a healthcare setting. You should also possess excellent communication and interpersonal skills, as well as the ability to work effectively in a team environment.
