Job Title: Hospice Care Nurse

The salary for this position is estimated to be around $80,000 - $100,000 per year, depending on experience.

About the Job:

RN Hospice, Fort Wayne, IN, Travel or Local. Mon-Fri., 8am - 5pm, 40 hours per week. Homecare Homebase (HCHB) EMR required, valid Nursing license and CPR.

Job Summary

The Registered Nurse (RN) will provide direct patient care services to hospice patients and supportive care to the patient's family; manage an interdisciplinary group consisting of Licensed Practical/Vocational Nurses, Certified Nursing Assistants and support staff to enhance the quality of hospice care; and, case manages all assigned patient clinical records.

  1. Assures the highest quality of care is provided and documented in accordance with the Plan of Care.
  2. Performs the initial and on-going assessment of the patient.
  3. Helps develop and implement the Hospice Plan of Care as prescribed by the physician and in conjunction with the other members of the Interdisciplinary Group.

We are seeking a skilled and compassionate Registered Nurse to join our team as a Hospice Care Nurse. This is a challenging and rewarding role that requires excellent communication and interpersonal skills.

Requirements

To be successful in this role, you will need to have a current nursing license in the state of Indiana and at least two years of experience as a registered nurse in a similar setting. You should also have strong knowledge of hospice care principles and practices, as well as excellent communication and interpersonal skills.
