Nursing Care Coordinator

Job Summary
As a Nursing Care Coordinator at Northwell Health, you will be responsible for delivering high-quality patient care by assessing and diagnosing patient conditions, developing plans of care, and evaluating patient responses. You will collaborate with interdisciplinary teams to coordinate and guide designated care team members in planning patient care and achieving goals.
Key Responsibilities:
- Develop and evaluate plans of care based on priority of patient problems, anticipated length of stay, input from patients and families, and other members of the healthcare team.
- Communicate pertinent information and changes in patient status to appropriate members of the care team and other healthcare providers as necessary.
- Collaborate with team members to make changes in the plan of care as necessary.
- Provide and/or delegate patient care according to the plan of care and established protocols.
- Conduct clinical rounds, provide emergency care, and administer medications to patients.
- Operate equipment required for patient care according to manufacturer's recommendations.
- Perform and assist with procedures and treatments on the unit, including but not limited to changing dressings, feeding patients, and providing pre/post-operative care.
- Promote and provide patient safety in accordance with state and federal guidelines.
- Document patient care as required on all appropriate forms.
- Enter date and time of entry and sign name in a legible manner on patient's chart.
Requirements:
- Graduate from an accredited School of Nursing.
- Current license to practice as a Registered Professional Nurse in New York State.
- BCLS, ACLS or PALS required.
