Job Overview

Northwell Health is seeking a skilled Nurse Care Coordinator to join our team. As a key member of our interdisciplinary group, you will be responsible for coordinating and delivering high-quality patient care.

 

Salary: $80,000 - $110,000 per year

 

Job Description

We are looking for a dedicated and compassionate nurse with experience in Home Care or Hospice settings. The ideal candidate will have strong communication and organizational skills, as well as the ability to work effectively in a fast-paced environment.

 

Responsibilities:

  • Conduct pre-admission screenings and initial nursing assessments on new patients.
  • Develop individualized plans of care based on patient needs.
  • Provide ongoing evaluation and coordination of patient care services.
  • Supervise and train home health aides, homemakers, and personal care workers.

 

Required Skills and Qualifications

We require a graduate from an accredited School of Nursing and current registration and license to practice as a Registered Professional Nurse in New York State. A minimum of two years of full-time nursing experience in Home Care or Hospice settings is also required.
