Key Position Details:
This role will primarily be remote, with occasional in-person patient consultations and interdisciplinary team meetings.
Call Rotation: 1 in 5 nights + 1 in 4 weekends
Job Description:
The physician will be responsible for delivering direct patient care by assessing medical histories and gathering, prioritizing, and synthesizing essential data relevant to the patient's health status. This includes offering education, consultation, or intervention to patients and their families regarding treatment plans.
The role involves the development, implementation, and evaluation of programs and systems that support individual patient and family care concerning symptom management, physical health, psychological support, and end-of-life planning. The physician will provide evidence-based care for patients with advanced progressive illnesses while fostering a healing environment.
Principal Responsibilities:
- Direct Patient Care:
- Assess and synthesize comprehensive patient data.
- Evaluate and coordinate information to inform decisions regarding patient care.
- Facilitate interdisciplinary care for symptom management and psychological support.
- Deliver evidence-based care for patients with advanced illnesses.
- Educate and consult with patients and families on treatment plans.
- Collaborate with patients to enhance their health and well-being.
- Practice with consideration for cultural diversity and legal responsibilities.
- Document care in electronic format as per licensure and scope.
- May provide care in various clinical settings.
- Participate in on-call rotation as needed.
- Quality Improvement:
- Utilize quality improvement tools to enhance patient care delivery and outcomes.
- Engage in peer review and quality assurance activities.
- Other duties as assigned.
Required Qualifications:
- Doctoral/Ph.D. level advanced degree in Medicine or Osteopathic Medicine from an accredited institution.
- Completion of an accredited Residency training program in Family Medicine or Internal Medicine.
- Completion of a 12-month training program in Hospice and Palliative Medicine or eligibility for certification examination.
- 0 to 2 years of relevant experience in the specialty area.
Licenses/Certifications:
- Licensed Physician in the state of Minnesota upon hire.
- ACLS-BLS Tier 3 certification required upon hire.
- Licensed Physician in the state of Wisconsin upon hire.
- Board Certification or Board Eligibility in relevant medical specialties required upon hire.
- Board Certification or Board Eligibility in Hospice and Palliative Medicine required upon hire.
- Drug Enforcement Agency Certificate required upon hire.
- Prescription Monitoring Program registration required upon hire.
- National Provider Identifier required upon hire.
Physical Demands:
- Medium Work: Lifting weight up to 32 lbs. occasionally and up to 25 lbs. frequently.
