We're seeking a highly skilled Patient Transition Coordinator to join our team at TotalMed in Lexington, Kentucky. As a Patient Transition Coordinator, you will be responsible for coordinating the transition of patients from intensive care to general medical-surgical units. The estimated salary for this position is $2,060.37 per week.

You will work closely with the interdisciplinary team to assess patients' needs and develop comprehensive care plans. Your responsibilities will also include monitoring vital signs, administering medications, and educating patients and families on post-intensive care recovery. To succeed in this role, you should possess strong communication and adaptability skills, with at least two years of recent Step Down experience.
