**Job Description:**

The successful candidate will deliver primary health care and patient care management in a residential setting, focusing on elderly individuals and those with complex medical issues.

**Responsibilities:**

- Provide comprehensive primary care to high acuity patients in their homes, Monday through Friday

- Manage patient care plans, ensuring seamless transitions and optimal outcomes

- Collaborate with interdisciplinary teams to coordinate care and address patient needs

- Utilize state-of-the-art technology and mobile diagnostic services to enhance patient care

**Qualifications:**

- Active, unrestricted medical license and DEA certification

- Board certification or eligibility in a relevant specialty (Geriatrics preferred)

- Experience with electronic medical records (EMRs)
