**Job Summary**

CHRISTUS Health is seeking a highly skilled Family Practice/Primary Care Nurse Practitioner to join our team in Wimberley, Texas. As a key member of our healthcare team, you will provide high-quality patient care and work collaboratively with our physicians and staff to deliver exceptional patient outcomes.

**Responsibilities:**

  • Provide primary and preventive care to patients of all ages, including health promotion, disease prevention, and management of acute and chronic illnesses.
  • Conduct thorough health assessments, develop and implement individualized care plans, and coordinate patient care with other healthcare professionals.
  • Order and interpret diagnostic tests, prescribe medications, and perform procedures as needed.
  • Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination.
  • Participate in quality improvement initiatives and stay up-to-date with the latest clinical guidelines and best practices.

**Requirements:**

  • Master's degree in Nursing or related field.
  • Current licensure as a Nurse Practitioner in the state of Texas.
  • Board certification in Family Medicine or related specialty.
  • Minimum 2 years of experience as a Nurse Practitioner in a primary care setting.
