Position Overview: The Clinical Care Coordinator will be responsible for:
- Setting and achieving departmental objectives.
- Establishing and maintaining effective policies and procedures.
- Coordinating services across various departments to ensure seamless patient care.
- Assessing staffing needs and ensuring a competent workforce.
- Conducting competency evaluations and ongoing training for staff.
- Monitoring quality metrics and implementing strategies to meet organizational goals.
Qualifications: Candidates should possess a minimum of three years of experience as a Registered Nurse in an acute care setting, with a background in Case Management or Utilization Management preferred.
Licensure/Certifications: A valid Registered Nurse license is required. Certification in Case Management is advantageous but not mandatory.
