Job Title: Licensed Clinical Social Worker

Job Summary:

We are seeking a highly skilled Licensed Clinical Social Worker to join our team at Gables Search Group. As a key member of our healthcare team, you will be responsible for providing high-quality patient care and coordination services to ensure optimal health outcomes.

Responsibilities:

  • Conduct complex psycho-social assessments and interventions to promote timely throughput, safe discharge, and prevent avoidable readmissions.
  • Develop and implement individualized transition plans to meet the unique needs of each patient.
  • Collaborate with healthcare professionals, patients, and families to ensure seamless care coordination and communication.
  • Participate in departmental Quality Improvement initiatives and contribute to the development of policies and procedures.
  • Provide education and support to patients, families, and caregivers to promote health literacy and self-advocacy.

Requirements:

  • Current licensure as a Licensed Clinical Social Worker (LCSW) in the state of California.
  • Minimum of two years of experience in acute hospital settings, preferably in case management or social work.
  • Excellent communication, organizational, and problem-solving skills.
  • Ability to work effectively in a fast-paced environment and prioritize multiple tasks and responsibilities.
