We are one of the largest medical practices in New England, with numerous locations across Connecticut and Rhode Island. Our diverse team includes professionals ranging from Medical Assistants to Physician Assistants, all dedicated to providing high-quality healthcare services across various specialties, including primary and urgent care.

Role of the Advanced Practice Provider

The Advanced Practice Provider (APP) will operate in an outpatient environment, focusing on the assessment and management of urology patients, particularly those with urgent or elective kidney stone issues. Responsibilities include patient counseling on prevention strategies, scheduling surgical interventions when necessary, and coordinating comprehensive care with urologists, nephrologists, and other healthcare professionals.

Our APPs lead patient care discussions with the healthcare team to adjust care plans as needed. They actively participate in quality management initiatives aimed at improving patient outcomes. This role offers the flexibility to cover various practice locations, adhering to a Monday through Friday schedule without on-call duties.

Key Responsibilities

  • Deliver direct patient care during office consultations, including assessments, triage, and emergency interventions.
  • Document health histories, conduct physical examinations, and order diagnostic tests.
  • Provide patient education regarding diagnoses and treatment plans.
  • Collaborate with physicians to implement appropriate care interventions within the APP's scope of practice.
  • Utilize clinical judgment based on best practices and evidence-based medicine.
  • Prescribe medications tailored to patient needs and ensure follow-up care.
  • Facilitate timely retrieval of laboratory results and diagnostic reports.
  • Coordinate referrals for comprehensive care across various specialties.
  • Work closely with clinical and administrative staff to enhance patient care experiences.

Work Schedule

Full Time, Monday – Friday. Travel may be required between designated practice locations.

Qualifications

Advanced Practitioner Requirements

  • Master's degree in nursing
  • National Certification in Advanced Practice and current CPR certification
  • Active State of CT APRN license with prescriptive authority and current DEA number
  • Minimum of 2 years of experience (urology preferred)
  • Strong clinical management skills and ability to work collaboratively in a team
  • Customer-focused approach with a commitment to excellent patient care
  • Exceptional written and verbal communication skills; proficiency in additional languages is a plus
  • Highly organized with the ability to prioritize tasks effectively
  • Familiarity with medical software systems, particularly Epic
  • Adherence to HHC MG policies, HIPAA regulations, and infection control standards
  • Ability to establish effective working relationships and maintain a positive attitude
  • Demonstrated initiative and proactive approach to achieving goals
