As one of the largest medical practices in New England, Hartford HealthCare Medical Group comprises a diverse team of healthcare professionals, including Medical Assistants and Physician Assistants. Our physician-led group is renowned for its dedication to patient care, offering a wide range of services from primary care to urgent care and over 30 specialties.
Role Responsibilities
The Advanced Practice Provider (APP) will operate in an outpatient environment, focusing on the assessment and management of urology patients with urgent and elective kidney stone issues. Responsibilities include:
- Conducting patient evaluations and follow-ups post-emergency care and surgical procedures.
- Providing guidance on kidney stone prevention and coordinating necessary surgical interventions.
- Collaborating with urologists, nephrologists, and other healthcare professionals to ensure comprehensive patient care.
- Leading patient care meetings to adjust treatment plans as necessary.
- Engaging in quality management initiatives to enhance patient outcomes.
Direct Patient Care
The APP will be responsible for:
- Performing office consultations, including assessments, triage, and emergency evaluations.
- Documenting health histories and physical examinations, along with ordering and interpreting diagnostic tests.
- Providing patient education regarding diagnoses and treatment plans.
- Collaborating with physicians to implement direct care interventions.
- Prescribing medications as needed and ensuring effective patient follow-up.
- Coordinating care with clinical and non-clinical staff.
Work Schedule
This is a full-time position with a Monday to Friday schedule. Travel between various practice locations may be required.

Qualifications
Required Credentials
- Master's degree in nursing.
- National Certification in Advanced Practice and current CPR certification.
- Active State of CT APRN license with prescriptive authority.
- Minimum of 2 years of relevant experience, preferably in urology.
- Strong clinical management skills and ability to work collaboratively.
- Excellent communication skills, with a focus on patient-centered care.
- Proficiency in medical software systems.
