Job Summary

We are seeking a skilled RN Field Care Coordinator to join our team at UnitedHealth Group. In this role, you will work in the field to coordinate the long-term care needs of patients in the local community.

Key Responsibilities:
  • Assess, plan, and implement care strategies tailored to individual patient needs
  • Identify and initiate referrals for social service programs, including financial, psychosocial, community, and state supportive services
  • Manage care plans throughout the continuum of care as a single point of contact
  • Communicate with stakeholders to ensure quality coordinated care and services are provided expeditiously to all members
  • Advocate for patients and families as needed to ensure their needs and choices are fully represented and supported by the healthcare team
Requirements:
  • Current, unrestricted RN license in TN
  • 5+ years of experience working with MS Word, Excel, and Outlook
  • 4+ years of experience working within the community health setting in a healthcare role
  • Experience in long-term care, home health, hospice, public health, or assisted living
  • Willingness or ability to travel in assigned region to visit Medicaid members in their homes and/or other settings
Preferred Qualifications:
  • Certified Case Manager (CCM) experience
  • Experience with electronic charting
  • Case management experience
  • Experience or exposure to discharge planning
  • Experience in utilization review, concurrent review, or risk management
  • Experience in a telephonic role
  • Experience with arranging community resources
  • Background in managing populations with complex medical or behavioral needs
