At UnitedHealth Group, we're committed to simplifying the healthcare experience and creating healthier communities. As a RN Field Care Coordinator, you'll play a vital role in coordinating the long-term care needs for patients in the local community.

Key Responsibilities
  • Assess and Plan Care Strategies: Develop individualized care plans that cater to the unique needs of patients, ensuring they receive the most appropriate level of care.
  • Identify and Initiate Referrals: Recognize opportunities for social service programs and initiate referrals for financial, psychosocial, community, and state supportive services.
  • Manage Care Plans: Serve as a single point of contact for patients, managing care plans throughout the continuum of care.
  • Communicate with Stakeholders: Effectively communicate with all stakeholders to ensure quality coordinated care and services are provided expeditiously to all members.
  • Advocate for Patients: Advocate for patients and families as needed to ensure their needs and choices are fully represented and supported by the healthcare team.
Requirements
  • Current RN License: Possess a current, unrestricted RN license in Tennessee.
  • Experience and Skills: Have 5+ years of experience working with MS Word, Excel, and Outlook, as well as 4+ years of experience working within the community health setting in a healthcare role.
  • Long-Term Care Experience: Possess experience in long-term care, home health, hospice, public health, or assisted living.
  • Travel Requirements: Be willing or able to travel in assigned regions to visit Medicaid members in their homes and/or other settings.
Preferred Qualifications
  • Certified Case Manager (CCM): Possess certification as a Case Manager.
  • Electronic Charting Experience: Have experience with electronic charting.
  • Case Management Experience: Possess experience in case management.
  • Discharge Planning Experience: Have experience in discharge planning.
  • Utilization Review Experience: Possess experience in utilization review, concurrent review, or risk management.
  • Telephonic Experience: Have experience in a telephonic role.
  • Community Resource Arrangement: Possess experience in arranging community resources.
  • Complex Medical Needs: Have experience in managing populations with complex medical or behavioral needs.
